Long-standing mitral valve disease is associated with enlargement of left atrium (LA) as a compensatory mechanism. Giant left atrium is seen in only 19% of patients and such a low incidence is probably related to an early development of pulmonary hypertension. We report the case of a giant left atrium with big thrombus diagnosed during an echocardiography done systematically as part of an etiologic assessment of a stroke.
