Abstract:

The action (case) that distinguishes by (3) three canals in the mandibular second premolar has been found and concluded to be 0.46-0.5%. It has been reported that the incidence of three canals in mandibular second premolar were scarce situation, unlike 97.5% that the mandibular second premolar had (1) one root canal, and only 2.5% have (2) two canals. Effective endodontic treatment needs full understanding of morphology and anatomy related to the canal. That’s why there are several differentiations in the canal system. Although the scarcity prevalence of three canals in mandibular second premolar, we should be acquainted with these several variations, radiographical and clinical anatomy, and orifices place exactly. In fact, the basic aim of this issue is to apply the effective canal treatment of mandibular lower second premolar with three canals, in other words this action must be done to achieve the ultimate endodontic success. The patient here in this case is a 49 years old Saudi female with a clear medical history (healthy and medically fit to operate) was referred to the clinic to check and for endodontic treatment on the mandibular lower second premolar. The basic and core complaint of the patient was severe pain in lower teeth second premolar. The revelation of the clinical examination shows the mesial caries in lower second premolar tooth (#35). Within the official radiographic examination reveals periodontium and more than one root canal was suspected in tooth (#35). The tooth (#35) was diagnosed with (3) three canals in the mandibular second premolar. We check within the vitality test, the tooth (#35) was diagnosed with and show irreversible pulpits with asymptomatic apical periodontitis. It was decided to undergo endodontic therapy for the mandibular lower second premolar. Indeed, endodontic treatment in lower second premolar tooth (#35) with a variety of morphology is a great and effective task. Therefore, the internal morphology must be identified precisely to achieve successful treatment. To conclude, an effective and successful management of mandibular lower second premolar with three canals has been doneprecisely.
